Cannot add Netapp LUN on ESXI 5 host

We have netapp (NetApp Release 8.1.2P3 7-Mode: ) and  ESXi ( VMware ESXi 5.1.0 build-1021289)

I  created LUN/igroup  with os type vmware.  But when I tried to  Add Storage - Disk/LUN  it failed on creating the VMFS 5 FS

Any idea what may be wrong here ?
