Find all 4 paths going to a single host and a single LUN.

I am trying to build a script to show me ALL the active paths to a LUN attached to a host.   I need this because whenever there is maintenance I need to Open my 6.0 web client(yes 6.0) click the host, ManageTab, Storage,vmhba1.  Under Adaptor details, I go to paths and scroll up and down looking for dead paths.

I would have thought someone would have this but I am unable to find something that works and give me all the paths.

Does someone have something like this?
